Supportive Housing Case Management

iMPACT! Compassion Center offers individualized Supportive Housing services with the goal of increasing an individual’s quality of life. All services are personcentered according to preferences, contributions, and conditions.

iMPACT! Compassion Center Supportive Housing services begin by creating an environment of stability and end in longterm self-sufficiency. Affordable housing, combined with support services, provides individuals with essential tools for building the skills to end cycles of homelessness and instability.

Participants have individually designed goal plans with support services. Services include; access to mental health, tenant education, budgeting, along with many others.

iMPACT! Compassion Center is committed to rapid entry and an individualized approach to supporting both landlords and tenants through case management.

To qualify, the client needs to have Apple Health/state Medicaid, be homeless or at risk of losing their housing, and have either a mental health, substance abuse, or disability need.
